The parent or legal guardian of the child they wish to exempt from vaccination needs to contact the Wyoming Department of Health, Immunization Program at 307-777-7446 and request a religious exemption form to be mailed to you. They will mail you the form and it is this form that must be used. They will not allow any copies of any forms. Once you have the form, complete it and have it notarized. Then mail it back to the state (preferably by certified mail) and they will sign the form and mail a copy back to you. The signed copy from the state is the copy that you give to the school or daycare upon registration. This form is good for both daycare and school.
