The parent or legal guardian of the child they wish to exempt from vaccination needs to fill out the Request from Immunizations for Reasons of Conscience form and mail, fax, or hand deliver your request to the fax number/address on the form. The quickest way is to go online and request the form, the link is: https://webds.dshs.state.tx.us/immco/affidavit.shtm
The request will be processed and the form mailed one week from the receipt of the request. You will receive in the mail an official Texas Department of State Health Services affidavit form. This form must be notarized and submitted to school officials within 90 days from the date it was notarized.
This process needs to be done every two years. The Texas Department of State Health Services, Immunization Branch can be telephoned at 1-800-252-9152 if you need additional questions.
