The parent or legal guardian of the child they wish to exempt from vaccination must contact the local county health department. Tell them you wish to obtain a religious exemption form and they will schedule an appointment for you to come in and fill out the form (Religious Exemption From Immunization Form DHEC 1126). They will not send the exemption form to the parent or legal guardian. An appointment must be made and the exemption form filled out in person. The nurse will then fill out a form for the parent or legal guardian to give to the daycare or school.
