The parent or legal guardian of the child they wish to exempt from vaccination needs to state in writing that immunization would violate his or her sincere religious beliefs and give the written statement to the daycare upon registration. SCHOOL:
The parent or legal guardian of the child they wish to exempt from vaccination must contact the school nurse and sign the religious exemption section on the back of the green and white card.
