The parent or legal guardian of the child they wish to exempt from vaccination needs to obtain the philosophical or religious exemption form from the school or daycare facility. In the event the school or daycare does not have the exemption form, you may obtain an exemption form by contacting the Oklahoma State Department of Health, Immunization Service at 800-234-6196 and request a philosophical or religious exemption form be mailed to you. They will mail you the form and it is this form that must be used, regardless if the form is obtained from the Department of Health or from the school or daycare. .
Once the parent or legal guardian obtains the philosophical or religious exemption form, you need to write a brief description in section 3 of why you object to immunizations, check which immunizations you do not want your child to receive and sign the form. After you complete this process, submit it to the daycare or school upon registration.
