The parent or legal guardian of the child they wish to exempt from vaccination needs to contact the Montana Department of Public Health and Human Services, Immunization Program at 406-444-4542 and request a religious exemption form be mailed to you. They will mail you the form and it is this form that must be used. They will not allow any copies of any forms. Once you have completed the form, have it notarized, and mail it back to the state (preferably by certified mail). This must be done every school year (not calendar year) in order for your child to be exempt from immunizations.
