The parent or legal guardian of the child they wish to exempt from vaccination needs to obtain the philosophical exemption form from the school. In the event the school or daycare does not have the form, you can download the form by clicking on the Minnesota Philosophical Vaccine Exemption Form link found below. Once the parent or legal guardian obtains the form, complete the “conscientious exemption” section, have it notarized, and submit it to the daycare or school upon registration.
