The parent or legal guardian of the child they wish to exempt from vaccination needs to fill out the philosophical or religious exemption form and give it to the school or daycare upon registration. In the event the school or daycare does not have the form, you can download the form by clicking on the Michigan Philosophical/Religious Vaccine Exemption Form link found below. Just note if the objection is due to philosophical or religious reasons in the “reason section” on the exemption form. The form is good for both daycare and school.
