The parent or legal guardian of the child they wish to exempt from vaccination needs to obtain the philosophical or religious exemption form from the school or daycare. In the event the school or daycare does not have the form, you can download the form by clicking on the Idaho Philosophical Vaccine Exemption Form link found below. Once the parent or legal guardian obtains the form, complete it and submit it to the daycare or school upon registration. The form is good for both daycare and school.
