The parent or legal guardian of the child they wish to exempt from vaccination needs to state in writing that immunization would violate his or her sincere religious beliefs and give the written statement to the school or daycare upon registration. There is no standardized form, (Click below on the link Georgia Vaccine Exemption Form.) only a written document from the parent or legal guardian. The document must be notarized.
