The parent or legal guardian of the child they wish to exempt from vaccination needs to obtain a religious or personal exemption form from the school or daycare. The forms can also be downloaded for free at the Colorado Department of Public Health and Environment, Immunization Program at: http://www.cdphe.state.co.us/dc/Immunization/Forms/immforms.html They can also be telephone at 303-692-2652.
Just sign on the form whether you want religious or personal exemption and give the form to the daycare or school upon registration.
Daycare is a little different. If your child is in a private daycare than the immunization issue is between the parent and the daycare. If your child is in a public affiliated daycare than the philosophical exemption applies to daycares the same as it does to public schools.
