The parent or legal guardian of the child they wish to exempt from vaccination must contact the local county health department. Tell them you wish to obtain a religious exemption form and they will schedule an appointment for you to come in and fill out the form. They will not send the exemption form to the parent or legal guardian. An appointment must be made and the exemption form filled out in person.
